Town of Cottage Grove Dog License Application for calendar year 2024
This form is for licensing NEW dogs.  To renew an existing license, go to http://renewdog.towncg.net.

Payment can be made by credit or debit card at the end of the form submission.  (A 2.95% convenience fee, minimum $1.00 will apply.  To avoid the fees, you may choose to mail a check.)

IF LICENSING MORE THAN ONE DOG, PAY FOR ALL OF THEM AT ONCE TO MINIMIZE THE PROCESSING FEE

Fees are as follows:
Adult Dogs (over 5 mos. of age on July 1, 2024)
    Neutered/Spayed.........................$18.25
    Un-Neutered/Un-Spayed..............$23.25
Puppies (under 5 mos. of age on July 1, 2024)
    Neutered/Spayed Female............$16.75
    Un-Neutered/Un-Spayed..............$19.25
Multi-Dog License:  $53.25 (allows for up to 12 dogs, however keep in mind the Town has a limit of 5 dogs on residential parcels of 2 acres or less).  Pay this amount if it is less than the total individual fees above.
